times. Fees cover collection services; specimens are delivered and
shipped to their designated laboratory for processing.
Primary Compliance Statement
Phlebotomy in Motion is an independent specimen collection service and does not perform laboratory testing. All specimens are collected and transported to the laboratory designated on the patient’s laboratory requisition for testing and processing.
Order Requirement Statement
All specimen collections require a valid I.D. and laboratory order from a licensed healthcare provider unless the patient is utilizing a physician-authorized laboratory test kit provided by a third-party laboratory.
